The goal of BrookHaven’s Outpatient Rehabilitation program is to provide or continue your therapy with maximum mobility, performance, and communication skills. Whether you are coming straight from home, or continuing after an inpatient stay with us, our professional staff evaluates each patient’s functional abilities and then designs a customized rehabilitation program for each individual. At BrookHaven we treat the whole person, seeking to improve the patient’s quality of life physically, emotionally and socially.

Our full-time team of physical, occupational and speech therapists provide therapy up to six days per week using an interdisciplinary, goal-oriented approach for each patient. When the patient is ready for discharge a thorough home visit is conducted to identify any safety and special equipment concerns.
